Rework gingering as a proxy wrapper for the slave object.  Remove all the gingering globals and stray functions.  Type fixes for markets.

VSCode and typescript still do not understand assignments to window as
introducing of a global name. To make them happy, use assignment to
globalThis.

Allow the nurse or a medically skilled PC to directly cure a small amount of shortDamage every week on slaves assigned to the clinic, without incurring any longDamage or condition cost for doing so.

notable tweaks to chem, cut some condition damage due to tiredness and the introduction of surgeryDamage for surgical losses of health so player skill can get involved.
