Small fixes for orgasm

There was an issue where pawns would not actually orgasm when the orgasm meter hit 100, but instead would orgasm a bit early. This commit fixes that by calculating the orgasm meter based on not just the duration/sex_ticks, but also the orgasmstick. In addition, it changes the masturbation mechanic so that the pawn will orgasm exactly once, instead of multiple times
