Flesh Heap generates a wider variety of amputees, instead of just quads.

Makes heap slaves with various amputation conditions if seeExtreme is enabled, instead of just all or nothing.

Chance of getting an amputee is up slightly (3/8 vs 1/3), but chance of getting a quad amputee is down significantly (1/8 vs 1/3); the difference is made up with double and single amputees.

Fixes #3388 (closed) and removes TODO.
