CommitSurgery Work

Split CommitSurgery into 8 pieces in the following order:

1. Disability related surgeries
2. Boob surgeries
3. Butt surgeries
4. Tightening surgeries
5. Prostate/Vasectomy surgeries
6. Other cosmetic surgeries (most of these are face related surgeries, body hair and liposuction are the exceptions)
7. Waist/hip surgeries
8. Belly implant surgeries

Only one surgery may be chosen from each piece, and they're all locked behind a if (slave.health > 20 && surgeries.length < 3) check. So surgeries will only continue to be performed so long as the slave is healthy at a maximum of three per week. I'm open to small reorganizations/reorderings before merging. Fixes #588 (closed).
