Read changelog from CHANGELOG.md and insert into new releases

This will pull the changes from the changelog and put them in the description.

@Pregmodder Make sure you add the version to the changelog before adding the tag, there is no fallback in case the version isn't found. If that becomes a problem I'll add it, but I'm quite happy it works at all.
