Adds several new lines!

Various spelling and markup corrections, as well as replacement of HTML entity references that XML parsers choke on.

April XML update from a .txt given by FrostFireFive.

Shuffles/Consolidates the testing tables. Also names them the Testing Tables for clarity. Testing out names for the tables too, because creativity is fun.

April rework for testing by FrostFireFive

As they both have the fewest unique lines for a non-silent female character (195 each), Lux and April have been moved off the main roster and into the offline_opponents folder. Should a contributor wish to mount an attempt to revive either of them, they are welcome to make improvements and then submit her for the testing tables where she will be subject to the same sponsorship standards as any character wishing to enter the main roster.

April's xmls were relics. Before make_xml.py, people used to hand-make xmls, which is why they were filled with commented instructions. And if you needed extra angle brackets, the local blacksmith could hammer some into shape for you.

As such, April did not have a source text document. I used the Character Editor to generate one, then did some checks for accuracy. One thing the Character Editor doesn't pick up is the fe/male_heavy_masturbating lines, which are not a thing and don't show up in the validator as a phony case. Maybe remarking on this state was a planned feature from way back or something. I moved these lines over to their non-heavy counterparts.

April didn't have a victory line, had only one generic swap_cards line, and had a few other validation errors. These are all fixed. I added a trivial number of additional lines while I was in there.

This character's number of unique lines increased from 179 to 193, which means she's still on the chopping block for future removal. The fixes in this commit should make it easier for someone to mount an attempt to get April re-sponsored should they want her terrifying face to return to the live game.

This update includes characters from A through D

All released characters should now have a <release> tag indicating the
relative order in which they were officially released in the game.
WIP characters in the repo do not have this tag, but it should be added
when they are officially completed.
Release numbers were automatically added by an external script, which also
seems to have converted some tabs to spaces and moved some comments, but
there should be no change to the meta.xml data (other than the new <release>
tag) otherwise.
